Understanding LAP Flap Reconstructive Surgery

At Florida Breast Center, we offer a wide range of advanced breast reconstruction options tailored to each woman’s unique needs. The Lumbar Artery Perforator (LAP) Flap is an advanced breast reconstruction technique that uses soft, fatty tissue from the lower back – commonly referred to as the “love handle” area. This tissue offers a natural look and feel, making it an excellent option for patients who are not candidates for abdominal flaps like the DIEP.

LAP flap surgery is particularly beneficial for women who have had a prior tummy tuck, significant abdominal scarring, or insufficient abdominal tissue for reconstruction. It can also be used when a patient has already had a DIEP flap on one side and now needs reconstruction on the other breast.

The procedure involves harvesting skin and fat from the lower back, along with a small blood vessel, and transferring it to the chest to form a new breast. Because the blood vessel (pedicle) is shorter than in other flap types, a vascular graft from another part of the body is often used to support blood flow to the new tissue.

Patients typically experience less discomfort than with abdominal-based flaps, and the donor site scar is low on the back, easily concealed by undergarments. Many also appreciate the added benefit of improved contouring along the waist and upper buttocks.

Recovery usually includes a brief hospital stay, with walking encouraged shortly after surgery. Most patients return to work and light activity within 4 weeks, with full healing continuing over several months.

Most patients stay in the hospital for a few days for monitoring. You’ll be up and walking shortly after surgery, though you may feel tightness in your lower back. Pain is often less than with abdominal flaps. You’ll need to avoid heavy lifting for about 4 weeks, and most people can return to light activity or work around that time.

The scar from a LAP flap procedure is placed low on the back, near the waistline. It’s typically well-hidden by underwear or swimwear. Many patients also notice a more contoured appearance in the lower back and flank areas after surgery.

The LAP flap is ideal for women who are not candidates for abdominal-based procedures like the DIEP flap. This includes patients who have had a previous tummy tuck, significant abdominal scarring, or have already used abdominal tissue for reconstruction. It’s also a great option for women with adequate lower back tissue who prefer a natural reconstruction without implants.
